Work Environment

The Physical Therapist will work onsite at our state-of-the-art Rehabilitation Facility in Freehold, New Jersey. You will collaborate closely with a diverse team of healthcare professionals, including occupational therapists, speech therapists, and medical doctors. Our facility is equipped with the latest technology and resources to provide comprehensive rehabilitation services. The work schedule is Monday through Friday, allowing for a balanced work-life dynamic while contributing to our patients' recovery journeys.

Key Responsibilities

• Conduct comprehensive patient evaluations by reviewing medical histories, performing targeted assessments, and documenting findings to develop and communicate tailored treatment plans.
• Engage patients and their families in discussions regarding evaluation results and collaboratively set achievable rehabilitation goals.
• Implement treatment protocols effectively in accordance with patient evaluations and ongoing healthcare developments, demonstrating a responsive approach to individual patient needs.
• Consult with interdisciplinary healthcare professionals when necessary, and document any observed changes in patient conditions that may require modifications to the treatment plan or additional consultations.
• Manage discharge planning processes and follow-up care while keeping detailed and accurate records of patient interactions, outlining plans of care, progress notes, and discharge evaluations in line with evolving healthcare regulations.
• Exhibit strong time management skills by maintaining an organized schedule, fulfilling productivity expectations, and attending pertinent meetings in a timely manner.
• Contribute to departmental goals and objectives, ensuring compliance with all regulatory and accreditation standards.
• Pursue opportunities for continuing education and professional growth, and serve as a clinical instructor for students, sharing your expertise with the next generation of physical therapists.
• Promote a superior customer experience by being adaptable and responsive to the needs of our patients, enhancing accessibility to care.
• Participate in initiatives aimed at removing barriers to healthcare access, utilizing site-specific data to drive improvements in patient experience and outcomes.

Patient Population / Client Focus

You will work closely with a diverse clientele, including individuals recovering from surgeries, injuries, chronic pain conditions, and neurological disorders. Our patient-centered approach emphasizes respect, empathy, and individualized care plans, ensuring that each person receives the highest level of service tailored to their unique rehabilitation journey.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid Physical Therapy license in the State of New Jersey.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A commitment to continuous professional development and staying updated on best practices in physical therapy.
