Key Responsibilities\n

• Conduct thorough psychosocial assessments involving patient interviews, observations, and reviews of medical records to determine the emotional, social, and environmental factors impacting recovery.

• Offer counseling and emotional support tailored to patients and their families, helping them navigate adjustment issues arising from disability or illness.

• Collaborate with the rehabilitation team to formulate individualized treatment plans that encompass psychosocial considerations along with medical and therapeutic needs.

• Facilitate access for patients to community resources, such as support groups, vocational rehabilitation programs, and housing assistance, ensuring they receive comprehensive post-discharge support.

• Educate families and patients on available community resources, enhancing their understanding of post-rehabilitation services and promoting successful transitions to home care.

• Actively participate in discharge planning, working with healthcare teams to coordinate smooth transitions from rehabilitation to community settings.

• Document patient interactions and progress in compliance with relevant guidelines and regulations, utilizing electronic health record (EHR) systems to track treatments and outcomes.

You will primarily work with individuals in hospice care, many of whom are facing terminal illnesses or significant life changes. Qualifications\n

• A Master's Degree in Social Work from an accredited institution.
