Key Responsibilities\n

• Provide comprehensive psychotherapy and mental health services to patients facing terminal illnesses or other serious health challenges, employing techniques that are sensitive to the unique needs of each patient.

• Conduct thorough assessments of patients’ emotional, behavioral, and social functioning to identify treatment needs and develop personalized care plans.
\n
• Utilize evidence-based practices and culturally competent approaches to formulate treatment strategies aimed at alleviating symptoms of anxiety, depression, and post-traumatic stress disorder (PTSD) among patients.

• Engage in regular consultations with interdisciplinary teams, including physicians, nurses, and chaplains, to coordinate care that addresses the holistic needs of patients and their families.

• Maintain accurate documentation of all patient interactions and treatment plans using our EHR systems, ensuring compliance with organizational policies and state regulations.

• Participate in regular clinical supervision and team meetings to foster professional growth and enhance service delivery effectively.

Patient Population / Client Focus\n

The focus of this role is primarily on patients receiving hospice care, many of whom may be experiencing complex emotional reactions related to end-of-life issues. You will conduct sessions to facilitate coping strategies and to provide emotional support, enhancing the quality of life for patients and offering guidance to their families during critical moments.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Valid and current state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated expertise in mental health assessment and evidence-based interventions.

• Exceptional interpersonal and communication skills, with an emphasis on building rapport with patients and family members.

Compensation and Benefits\n
