Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients' mental health status, identifying strengths, needs, and available resources for support.

• Implement evidence-based therapeutic approaches such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to assist individuals in managing anxiety, depression, and other mental health issues.

• Collaborate seamlessly with psychiatrists, nurses, social workers, and other healthcare professionals to develop integrated treatment plans that address the holistic needs of patients.

• Educate patients and their families about mental health diagnoses, treatment options, and medication alternatives, facilitating informed decision-making.

• Provide individual, group, and family therapy to foster emotional resilience and coping strategies, utilizing best practices in a remote setting.

• Develop personalized treatment plans in collaboration with the treatment team, outlining clear goals and therapeutic milestones to achieve optimal outcomes.

• Support patients in navigating the complexities of the mental health system, serving as an advocate for their rights and access to services.

• Facilitate therapeutic groups and educational sessions, as well as take part in discharge planning meetings to ensure smooth transitions for patients.

Patient Population / Client Focus\n

In this role, you will primarily serve patients at various stages of life, including those who are facing the challenges of terminal illnesses or severe emotional distress. You will provide compassionate care to individuals and families coping with grief, loss, and the intricacies of mental health concerns related to both physical and emotional health. Your role will be critical in promoting wellness and ensuring that patients feel understood and supported throughout their journeys.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
