Work Environment\n

This position is 100% remote, offering you flexibility in your work schedule while providing essential services to clients. As a part of a compassionate team within a Hospice Center setting, you will be empowered to create your own schedule, accommodating evening and weekend hours as needed to best serve our clients. Your interactions will be facilitated through telehealth platforms, promoting a convenient and effective approach to treatment.

Key Responsibilities\n

• Utilize your advanced knowledge and training to independently develop psychosocial assessments for clients, applying evidence-based diagnostic techniques.

• Design comprehensive treatment plans that cater to individual client needs, adapting your approach as necessary based on the progress and feedback received.

• Employ a variety of therapeutic methodologies including solution-focused therapy, behavioral activation, and motivational interviewing to engage clients effectively.

• Work collaboratively with a multidisciplinary team consisting of healthcare professionals, primary care physicians, and psychologists to ensure a holistic treatment approach.

• Monitor and evaluate client progress towards the agreed therapeutic goals, providing feedback and adjusting treatment plans accordingly.

• Facilitate medication management in collaboration with medical providers, ensuring adherence to prescribed treatments while closely managing and discussing side effects.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid, and unrestricted LCSW license to practice independently in the state of California.
