Work Environment\n

This position is completely remote, allowing for flexibility while maintaining a supportive remote work culture. Digitally connect with your colleagues and interdisciplinary healthcare team members while providing services effectively to our hospice patients. All interactions and care management are facilitated through secure electronic health record (EHR) systems, enabling seamless communication and documentation.

Key Responsibilities\n

• Collaborate closely with the healthcare team to develop comprehensive treatment plans, ensuring patient-centered care by assessing psychosocial factors affecting health outcomes.

• Perform thorough evaluations of patients’ emotional and behavioral issues and craft individualized intervention strategies to address them.

• Maintain timely and accurate documentation, including clinical assessments and progress notes, in compliance with department policies and regulatory standards.

• Provide critical counseling and crisis intervention, helping patients cope with grief, anxiety, and other mental health challenges associated with end-of-life transitions.

• Facilitate communication by consulting with families and support systems about client treatment, discharge planning, and ongoing care.

• Empower patients by connecting them with community resources and support systems tailored to their unique circumstances.

Patient Population / Client Focus\n

This role focuses primarily on hospice patients facing terminal illnesses, requiring specialized knowledge of the emotional, social, and practical challenges they encounter. You will provide support not only to patients but also to their families, ensuring that compassionate care transcends the clinical environment.

Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited institution.

• Active and unrestricted LICSW licensure in the state of Washington.

• A minimum of three years of unsupervised, post-licensure experience working in mental health or social services.

• Proficient computer skills, including word processing, email communication, spreadsheets, and electronic medical records systems.

Compensation and Benefits\n
