Key Responsibilities\n

• Develop individualized support plans for clients, assisting them in coping with their conditions and improving their overall quality of life.

• Formulate and implement tailored treatment goals that are specific, measurable, achievable, relevant, and time-bound (SMART), monitoring and modifying them as needed based on client progress and feedback.

• Provide expert counseling and guidance during therapy sessions, utilizing a range of therapeutic techniques and approaches to address challenges and facilitate positive change.

• Maintain detailed and accurate records of client interactions, treatment plans, and progress, complying with relevant legal and ethical standards regarding confidentiality.

• Participate in case conferences, collaborating with healthcare professionals, and contribute to comprehensive care plans.

• Serve as a key worker, coordinating care with other service providers and community organizations to ensure a holistic approach to client recovery.

• Refer clients to local community resources and services to promote ongoing support beyond therapy sessions.

Patient Population / Client Focus\n

In this role, you will primarily work with patients who are facing terminal illnesses, providing emotional and psychological support tailored to their specific needs. You will also support families navigating the complexities of end-of-life care, addressing the psychological implications of grief and loss, and offering them the tools necessary to cope with the emotional aftermath.

Qualifications\n

• A Master’s Degree in Social Work (MSW) is required, with relevant licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.

Compensation and Benefits\n
