Key Responsibilities\n

• Conduct Psychosocial Assessments: Utilize your knowledge of human behavior and psychosocial factors to carry out thorough assessments in a timely manner. Address potential ethical and legal issues in all evaluations.

• Provide Clinical Interventions: Offer supportive counseling to patients and their families, while assisting with discharge planning and community linkage. Facilitate family conferences to aid families in making difficult decisions regarding end-of-life care, including navigating hospice options and Do Not Resuscitate (DNR) orders.

• Crisis Management: Efficiently manage your clinical caseload by assessing high-risk situations, balancing referrals, and collaborating with case managers to ensure smooth transitions of care.

• Maintain Ethical and Legal Standards: Uphold ethical standards in all clinical interventions, ensuring compliance with legal requirements related to patient rights and confidentiality.

• Documentation: Ensure meticulous documentation of assessments, treatment plans, and progress notes as per organizational policies, including evidence-based plans of care.

• Conduct In-Service Training: Share your knowledge and insights on psychosocial care with fellow team members via in-service trainings and lunch-and-learn sessions.

• Participate in Quality Improvement Initiatives: Engage in developing performance indicators to enhance the quality of care delivered to our patients.

• Supervise and Mentor: Provide supervision to graduate-level social work students and MSW employees, overseeing their clinical development and adherence to standards.

• Contribute to Community Programs: Actively support Social Work Month activities and lead educational programs for patients, families, and healthcare staff.

• Adhere to Organizational Policies: Follow all Tenet policies, procedures, and practices accurately while performing other duties as assigned.

This position focuses on a diverse patient population, including families and individuals facing complex psychosocial issues related to medical conditions, particularly within the realm of chronic illness and cancer care. You will work collaboratively with an interdisciplinary team to ensure holistic approaches to patient well-being, addressing both emotional and clinical needs.

Qualifications\n

• Master's degree in Social Work from an accredited graduate school.

• Minimum of 2 years of experience in a medical setting, with at least 1 year in a cancer care environment.

• Current California Licensure as a Clinical Social Worker (LCSW).
\n
• Basic Life Support (BLS) certification from the American Heart Association.
\n
• Proficient in verbal and written communication in English, with a strong emphasis on customer service.

• Demonstrated problem-solving and analytical skills, with the ability to work independently and as part of a team.

• Commitment to excellence in patient care and service.
\n

Preferred Experience\n

Compensation and Benefits\n

The annual salary for this position ranges from USD 120,000 to 130,000 , commensurate with experience and qualifications.
