Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic conditions in a home care setting, utilizing assessments to establish specific rehabilitative goals.
• Design and implement individualized treatment plans that incorporate methods such as therapeutic exercises, stretching techniques, hands-on therapy, and specialized equipment to alleviate pain and restore function.
• Monitor and document patient progress meticulously, adjusting treatment plans based on outcomes and feedback to ensure that goals are consistently met.
• Provide education and training to patients and their caregivers regarding treatment protocols, exercises, and the proper use of assistive devices to promote independence.
• Stay updated with the latest developments in physical therapy by participating in educational workshops, professional networking, and reviewing relevant literature.

Patient Population / Client Focus

The patients you will serve range from children to seniors, each facing unique orthopedic challenges. Qualifications

• Proficiency in computer skills and documentation processes is important to maintain accurate patient records.

Preferred Experience

• A minimum of two years of experience in outpatient or home health physical therapy, preferably with a focus on orthopedic rehabilitation.
• Experience with electronic medical records and a familiarity with applications used for patient documentation.
• Previous work with diverse populations, including pediatric and geriatric patients, is advantageous.

Compensation and Benefits
