Key Responsibilities\n

• Conduct clinical social work evaluations to assess patient needs, developing aftercare plans that facilitate successful transitions upon discharge.

• Monitor and document medication side effects in collaboration with psychiatrists, ensuring patients receive safe and effective treatment.

• Provide crisis intervention and emotional support to patients and families facing end-of-life issues, employing evidence-based counseling techniques.

• Maintain a detailed caseload, managing documentation such as biopsychosocial assessments, treatment plans, progress notes, and discharge summaries in compliance with organizational standards.

• Engage in case management and care coordination, leading treatment team meetings and participating in utilization reviews to optimize patient outcomes.

• Advocate for patients by researching and connecting them to community resources, including healthcare access, financial aid, and supportive services.

Patient Population / Client Focus\n

This position primarily serves individuals and families navigating the hospice process, often dealing with complex psychosocial issues related to grief, loss, and end-of-life decisions. Qualifications\n

• Master's Degree in Social Work from an accredited graduate program.

• Active and unrestricted California Clinical Social Worker License (LCSW).

Compensation and Benefits\n
