Work Environment

This position is based onsite at our outpatient office in Perth Amboy. Key Responsibilities

• Conduct thorough evaluations by reviewing patient histories, performing specific assessments, and developing comprehensive treatment plans tailored to individual needs.
• Communicate evaluation findings and treatment strategies effectively to patients and their families, ensuring involvement in the goal-setting process.
• Implement treatment protocols competently based on evaluation results and the patient's evolving condition, adapting as necessary in alignment with best practices.
• Collaborate with other healthcare professionals to coordinate care, promptly recognizing and documenting changes in patient conditions that necessitate adjustments in the treatment plan.
• Oversee the discharge planning process, ensuring thorough follow-up care and documentation that traces the patient's journey through the clinic, including initial evaluations and progress notes.
• Exhibit strong time management skills to meet productivity expectations, maintaining an organized schedule and punctual attendance for meetings and patient appointments.
• Engage in departmental team initiatives, contributing towards achieving established goals while adhering to compliance with all governmental and regulatory requirements.
• Pursue professional development through continuing education and mentorship opportunities, serving as a clinical instructor for students as needed.

Patient Population / Client Focus

You will primarily work with a diverse outpatient population, including individuals recovering from surgery, sports injuries, and chronic pain conditions. Qualifications

• A Bachelor's Degree in Physical Therapy is required, though candidates with a Doctorate in Physical Therapy (DPT) will be prioritized.
• A valid license to practice physical therapy in New Jersey is mandatory.
• Current American Heart Association BLS for Healthcare Providers CPR certification is required.

Preferred Experience

• A minimum of 2 years of prior experience in outpatient physical therapy is preferred but not mandatory for exceptional new graduates.
• Background in using electronic health record (EHR) systems to document patient care and manage schedules is beneficial.
