Key Responsibilities\n

• Conduct continuous assessments of patients to adequately tailor care to their evolving needs, ensuring effective support throughout their treatment journey.

• Maintain comprehensive communication with supervisors, direct care staff, and family members regarding patient progress, fostering a collaborative approach in treatment.

• Create and provide supervisors with a structured schedule detailing appointments, home visits, and necessary observations for effective care management.

• Organize and coordinate home visits to facilitate in-home observations, chart reviews, and staff interviews, ensuring thorough engagement with patients and their families.

• Monitor and ensure the execution of individualized care plans is carried out effectively, addressing any challenges that arise promptly.

• Participate actively in Interdisciplinary Team Meetings, contributing insights that enhance patient care and program effectiveness.

• Prepare concise monthly and quarterly progress notes to document patient developments and changes in treatment needs.
\n
• Organize and present training sessions to certify staff members on effective program plan implementation and accurate documentation practices.
\n
• Conduct follow-up interviews and home visits, ensuring ongoing support for patients to facilitate continuity of care and address their needs.

• Serve as a vital link between the public and mental health community, offering insights and strategies to aid in developing community-based support systems.

• Assist in creating effective training materials and procedural guides aimed at enhancing program development for individuals.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Clinical Counselor (LPCC) in the state of California.

Compensation and Benefits\n
