Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, considering emotional, mental, and social factors, to create individualized care plans.

• Collaborate with clients and their families to develop tailored strategies that address their specific goals, preferences, and challenges.

• Provide counseling and emotional support to seniors, guiding them through life transitions, crises, and other challenges they may face.
\n
• Serve as a strong advocate for clients’ rights, ensuring they have access to necessary community resources, support services, and entitlements.
\n
• Regularly monitor client well-being through home visits and virtual check-ins, addressing any safety concerns and offering continued support.
\n
• Facilitate effective communication among family members involved in care, helping to navigate conflicting perspectives and fostering cooperative decision-making.
\n
• Educate clients and their families about available community resources, programs, and support groups, enhancing their engagement and connection to the outside world.

• Document client assessments, care plans, progress notes, and interactions meticulously, ensuring accuracy and compliance with organizational policies.

• Maintain up-to-date knowledge of social work practices and regulations, understanding ethical guidelines to uphold the professionalism of the role.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive annual salary ranging from $60,000 to $100,000, depending on experience and qualifications.
