Foundation Orthopedics, P.C. is seeking a dedicated and skilled Full-Time Home Health Physical Therapist to join our team in Browns Mills, New Jersey. Work Environment

This position is onsite and primarily requires travel to our clients' homes around Browns Mills, NJ. As a member of an interdisciplinary team, you will work closely with other healthcare professionals to promote holistic care. Key Responsibilities

• Direct and guide treatment for patients by designing personalized physical therapy plans that aim to prevent disability, alleviate pain, and restore optimal muscle function.
• Oversee the work of physical therapy assistants to ensure the effective execution of care plans, providing mentorship and guidance when necessary.
• Conduct thorough evaluations of clients' functional abilities using specialized diagnostic and prognostic tests including muscle strength, joint mobility, and functional performance assessments.
• Assess home environments to determine necessary adaptations that promote patient independence and safety during rehabilitation.
• Deliver prescribed physical therapy services directly in patients' homes, ensuring adherence to physician orders and treatment protocols.
• Administer manual therapy techniques and instruct clients in therapeutic exercises, motivating them to engage fully in their rehabilitation process.
• Evaluate, fit, and recommend modifications for prosthetic and orthotic devices, collaborating with physicians to enhance the patient's recovery journey.
• Contribute to interdisciplinary team meetings, collaborating with healthcare professionals on comprehensive care plans while sharing insights regarding patient progress and possible adjustments.
• Maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, for each client.
• Instruct clients and their families on physical therapy regimens and the importance of adherence to achieve maximum recovery.
• Participate in ongoing education and skill development to maintain and enhance clinical knowledge and best practices in physical therapy.

This role primarily focuses on a diverse patient population, including individuals recovering from surgery, those with chronic pain conditions, elderly patients seeking mobility assistance, and individuals requiring post-hospitalization rehabilitation. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• State licensure to practice as a Physical Therapist in New Jersey, with an impeccable record free of sanctions or board proceedings.
• Demonstrable expertise in various manual therapy techniques and the formulation of effective exercise programs.
• Proficient in electronic medical records (EMR) software, especially WebPT, to maintain precise patient documentation.
• Current knowledge of evidence-based physical therapy techniques and trends.

Preferred Experience

• Experience in home healthcare settings, particularly with geriatric populations and chronic conditions.
• Familiarity with community resources and rehabilitation programs that can support patient recovery in their home environment.
• Experience collaborating with an interdisciplinary healthcare team to improve patient outcomes.

Compensation and Benefits
