Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for infants and toddlers, integrating recommendations into actionable care plans.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs) in collaboration with families and multidisciplinary teams.
\n
• Monitor progress and outcomes, ensuring interventions are aligned with best practices and regulatory guidelines.

• Provide targeted support and guidance to families, equipping them with the skills necessary for optimal child development.

• Adhere to all Federal and State regulations governing early intervention services, including reporting and documentation requirements.
\n
• Build effective relationships with children and their families, creating a nurturing environment that promotes learning and development.
\n
• Design individualized treatment strategies grounded in evidence-based practices and Natural Learning Environment Principles.
\n
• Advocate for families, providing resources and support systems to facilitate child and family success.
\n
• Utilize a coaching model and natural environment teaching to engage families in the therapeutic process.

Patient Population / Client Focus\n

In this role, the LCSW will primarily work with infants and toddlers facing disabilities or developmental delays who are receiving care through a hospice center. This vulnerable population requires tailored interventions that not only address their medical needs but also support emotional and psychological well-being. Qualifications\n

• A Master’s Degree in Social Work or a related discipline.
