Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing medical history, performing specific assessments, and documenting results, ensuring a thorough understanding of each individual's needs.
• Communicate evaluation findings and develop personalized treatment plans in collaboration with patients and their families, emphasizing shared goal-setting.
• Implement evidence-based treatment protocols tailored to each patient's condition, adapting approaches based on ongoing assessments and changing healthcare environments.
• Consult with interdisciplinary healthcare teams to ensure cohesive patient care; identify and document any changes in patients' conditions that warrant modifications to treatment plans.
• Oversee discharge planning to ensure patients have the resources and support needed for successful transitions; ensure proper record-keeping that reflects the patient's journey, including initial evaluations, daily notes, interim progress assessments, and discharge summaries.
• Meet departmental productivity expectations while effectively managing time, maintaining an organized schedule, and participating in meetings punctually.
• Contribute to departmental goals and comply with all relevant regulations and accreditation standards.
• Engage actively in continuing education to not only enhance your personal skill set but also impart knowledge to others by serving as a clinical instructor for students.
• Deliver exceptional customer service, demonstrating flexibility and responsiveness to patient needs.
• Participate in initiatives focused on improving access to care, including teamwork to dismantle barriers in our community.

Patient Population / Client Focus

The physical therapist will engage with a diverse patient population, including individuals recovering from surgeries, accidents, or chronic conditions requiring rehabilitation. This role requires compassionate communication and a commitment to understanding the unique needs of each patient, ensuring they receive personalized care that caters to their specific rehabilitation goals.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Must possess a valid DPT license in the State of Florida.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is essential.
• A strong commitment to professional development and learning is necessary.

Preferred Experience

• While a minimum of two years of relevant experience is preferred, we welcome all driven candidates who hold the required credentials.
• Experience in an outpatient rehabilitation setting would be advantageous, although not mandatory.
