Work Environment

This position is primarily onsite, providing home care services within Fairview, New Jersey. Our therapists utilize a patient-centered approach, allowing for flexible scheduling with the autonomy to select hours that best suit their work-life balance. Key Responsibilities

• Conduct initial evaluations of patients in their homes, assessing their physical condition, mobility limitations, and therapeutic needs.
• Develop personalized treatment plans that address specific patient goals, utilizing techniques such as therapeutic exercises, manual therapy, and the use of specialized equipment.
• Implement and adapt treatment modalities based on patient progress and feedback, ensuring effective interventions to facilitate recovery.
• Regularly document patient progress, treatment responses, and any changes in their condition to uphold high standards of care and facilitate communication with other healthcare professionals.
• Provide education and training to patients and their caregivers on exercise routines, health management strategies, and the proper use of assistive devices.
• Continuously evaluate patient outcomes and modify treatment plans as necessary to promote optimal recovery and wellness.
• Engage in professional development activities such as workshops and seminars to stay updated with the latest practices in physical therapy.

Patient Population / Client Focus

Our team serves a diverse patient population, including children recovering from surgeries or injuries, elderly patients dealing with mobility issues, and individuals seeking rehabilitation from chronic pain or illness. Each therapist plays a critical role in customizing treatment plans to meet the unique needs of every patient, ensuring that care is both effective and empowering.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid physical therapy license in the state of New Jersey.
• Excellent verbal and written communication skills for effective patient interaction and documentation.
• Competency in using word processing software and other technology essential for patient management.
• Demonstrated ability to be flexible and adapt to the dynamic needs of home care environments.

Preferred Experience

• Minimum of 2 years of experience in physical therapy, preferably in a home care setting, is highly desirable.
• Experience working with diverse populations, including pediatrics and geriatrics.

Compensation and Benefits
