Key Responsibilities\n

• Plan and implement individualized support packages that help clients address and navigate their unique challenges, fostering resilience and coping skills.

• Develop tailored treatment plans with measurable goals that reflect the clients' needs and preferences.

• Continuously monitor and evaluate clients' progress, making necessary adjustments to treatment approaches to ensure optimal outcomes.

• Provide evidence-based counseling and information during sessions, helping clients navigate through decision-making processes.

• Maintain detailed and accurate client records, documenting all interactions and treatment progress in compliance with legal and organizational standards.

• Participate in case conferences and collaborate with multidisciplinary teams to ensure a coherent approach to client care.

• Act as a key point of contact and advocate for clients, facilitating referrals to community resources to promote client recovery and well-being.

You will primarily work with clients who are in hospice care, facing significant life transitions. This population demands a compassionate and empathetic approach as they deal with grief, loss, and complex healthcare decisions. Qualifications\n

• A Bachelor's Degree in Social Work (BSW), Psychology, or a related field is required. A Master’s degree (MSW) is preferred.

• Active licensure as a Clinical Social Worker is mandatory.
