Work Environment

This position is primarily onsite, providing home care services to patients throughout Anaheim, California. As a Home Care Specialist, you will travel to various locations, creating a flexible schedule that meets the needs of our patients and ensures they receive the care they deserve. Working independently in patients' homes, you will facilitate a comfortable and familiar setting for rehabilitation, which can enhance recovery outcomes.

Key Responsibilities

• Develop and implement personalized treatment plans for patients with orthopedic conditions, ensuring they receive comprehensive care tailored to their specific needs.
• Administer physical therapy treatments utilizing a variety of techniques, including therapeutic exercises, manual therapy, and modalities such as heat or cold therapy.
• Guide and instruct patients through functional activities including bed mobility, transfers, walking, and balance exercises, while closely monitoring their progress.
• Utilize assistive devices such as walkers or canes to aid patients in improving mobility and overall function.
• Educate patients and their families about treatments and exercises to be performed independently, fostering a proactive approach to recovery.
• Maintain thorough documentation of patient progress and treatment outcomes, ensuring compliance with legal and organizational standards.
• Collaborate with licensed physical therapists to report patient status and adjust treatment plans as necessary.
• Participate in training and mentoring staff, sharing best practices and clinical insights to enhance team performance.

Patient Population / Client Focus

As an Orthopedic Physical Therapist, you will work primarily with patients recovering from orthopedic surgeries, sports injuries, and musculoskeletal disorders. Your role will involve assessing individual needs and tailoring interventions to meet specific goals, helping each patient regain strength and mobility. Your expertise will play a critical role in enhancing the functional capabilities of patients in their everyday lives, maximizing their independence and well-being.

Qualifications

• Current California Physical Therapist Assistant (PTA) license is required.
• A minimum of a Bachelor's Degree in Physical Therapy or a related field is essential.
• At least 2 years of experience in a clinical setting, preferably with a focus on orthopedic care.
• Strong communication and interpersonal skills to effectively engage with patients, families, and team members.

Preferred Experience

• Experience providing in-home physical therapy services to a diverse patient population.

• Experience in mentoring or training other clinical staff is a plus.

Compensation and Benefits
