Qualifications Certifications: Licensed Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA), Education: Bachelor's Degree: Physical Therapy (Required), Work Experience: Responsibilities Reviews requisitions for physical therapy and evaluates patient's physical condition and needs.Establishes PT treatment goals for each individual patient, based on the findings of the initial a program of corrective exercise and treatment for assessed the treatment and recording of patient's progress.Determines proper equipment usage, instructs the patient in appropriate program, and modifies program as needed.Teaches patient, other staff members, and student affiliates in passive, active assistive, or resisted exercise, ambulation, and other physical movements in order to articulate and demonstrate principles of rehabilitative therapy.Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.Enhances professional growth and development through participation in educational programs, current literature, workshops, and inservice meetings.Documents as required by the profession and department, such as evaluation results, treatment plans, progress reports, and other such reports.Collaborates with support personnel according to legal standards and ethical guidelines.Establishes a PT Plan of Care based on functional deficits and patient goals. Completes objective testing for range of motion, strength, and performance of daily functional tasks. Completes therapeutic modalities, as appropriate, including ultrasound, iontophoresis, and electrical stimulation. Administers corrective exercise or manual techniques for stabilization, facilitation, or inhibition of movement. Completes daily documentation and charging in Therapy EMR. Communicates with the referring provider on patient progress and treatment recommendations. Provides supervision to Physical Therapy Assistants, in accordance with regulatory agency guidelines. Compensation and Benefits The compensation range for this position is $36.74per hour - $63.19per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
