Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough psychosocial assessments through patient interviews, observations, and analysis of medical records to understand the emotional, social, and environmental challenges that may hinder recovery.

• Providing empathetic counseling and support to patients and their families, helping them navigate emotional turmoil and adjustment issues related to their health conditions.

• Working collaboratively with a multidisciplinary rehabilitation team to create personalized treatment plans that address the unique psychosocial needs of each patient.

• Facilitating access to essential community resources such as support groups, vocational rehabilitation services, and housing assistance, thus empowering patients to take charge of their recovery.

• Educating patients and their families about available community resources and post-rehabilitation services to ensure seamless transitions and sustained support.

• Participating actively in discharge planning, making sure each patient's transition to home or other care settings is well-coordinated and effective.

Patient Population / Client Focus\n

Our clinical focus primarily serves individuals rehabilitating from severe medical conditions, such as strokes, spinal cord injuries, and neurological disorders, as well as patients recovering from orthopedic surgeries. You will play a critical role in addressing the multifaceted emotional and social needs of these patients, promoting their reintegration into the community.

Qualifications\n

The ideal candidate for this position will have the following qualifications:

• A Master's degree in Social Work from an accredited program.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated proficiency in understanding adjustment issues, grief, coping mechanisms, and their impact on patient recovery.
