Work Environment

Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities, functional limitations, and medical histories to develop effective treatment plans.
• Implement and monitor individualized rehabilitation programs, adapting interventions based on patient progress and ongoing assessments.
• Provide consultation services to other healthcare providers regarding patient care, ensuring continuity in treatment.
• Educate patients and their families about the therapeutic process, empowering them to take an active role in their recovery.
• Maintain accurate documentation of patient evaluations, treatment plans, and progress, in compliance with industry regulations and best practices.
• Collaborate with a multidisciplinary team, including physicians, occupational therapists, and nursing staff, to ensure comprehensive and coordinated care.
• Participate in community outreach initiatives, program development, and continuous quality improvement (CQI) efforts.
• Attend regular staff meetings and continue personal and professional development through ongoing education and training opportunities.

Patient Population / Client Focus

Your role as a Physical Therapist at Touchstone Health Therapy will involve working closely with patients who have orthopedic impairments, including post-surgical rehabilitation, sports injuries, and age-related musculoskeletal conditions. You will interact with individuals of all ages, tailoring your approach to meet the specific needs of each patient and family, ensuring a respectful and culturally sensitive practice.

Qualifications

The ideal candidate will possess the following qualifications:

• Bachelor's Degree in Physical Therapy from an accredited institution; a Doctorate in Physical Therapy is preferred.
• A valid State License to practice Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong verbal and written communication skills, with proficiency in word processing and familiarity with electronic health record (EHR) systems.
• A commitment to maintaining compliance with all applicable laws and regulations relevant to patient care.

Preferred Experience

While not mandatory, candidates with prior experience in home health physical therapy or working with diverse patient populations will be considered favorably. Experience in community outreach or leadership roles is an added benefit.

Compensation and Benefits
