Key Responsibilities\n

• Conduct thorough mental health evaluations to assess the emotional and psychological needs of patients, considering their co-occurring medical conditions.

• Develop and implement personalized treatment plans that address both psychiatric issues and physical health factors, ensuring a holistic approach to patient care.

• Facilitate individual and group therapy sessions using evidence-based methodologies tailored to patient needs.

• Coordinate with primary care and specialty medical providers to ensure continuity and effectiveness of care.

• Provide immediate crisis intervention and support, effectively handling mental health emergencies with compassion and professionalism.

• Empower patients through education on the relationship between mental and physical health, as well as self-management strategies for their conditions.

• Maintain accurate and secure documentation in compliance with state and federal regulations, ensuring that confidentiality is prioritized.

• Actively participate in clinical meetings, case discussions, and collaborative care planning to share insights and enhance treatment outcomes.

Patient Population / Client Focus\n

Our outpatient services focus on a diverse range of patients, many of whom present with complex needs due to co-occurring medical and mental health conditions, such as autoimmune disorders and substance use issues. As a therapist, your expertise will be invaluable in navigating these multifaceted challenges, and you'll work closely with patients to improve their overall quality of life.

Qualifications\n

• Possession of a Master's or doctoral degree in Social Work, Clinical Psychology, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or equivalent in New York state.
\n
• Proven experience in treating patients with complex mental health and physical health needs, preferably within an integrated care framework.

• Strong interpersonal communication skills and a commitment to maintaining professional boundaries and ethical practices.

• Competence in using electronic health record systems and related clinical applications.
\n

Preferred Experience\n

While not required, experience with evidence-based therapies, group facilitation skills, and knowledge of crisis intervention strategies will be beneficial in this role. Familiarity with Telehealth systems will also enhance your ability to connect with patients effectively in a remote environment.

Compensation and Benefits\n
