Key Responsibilities\n

• Conduct thorough psycho-social assessments to diagnose and treat a wide range of mental health disorders.

• Develop and implement individualized treatment plans, modifying them as necessary based on patient progress and feedback.

• Collaborate with multi-disciplinary teams to create comprehensive discharge plans, ensuring continuity of care as clients transition out of therapy.

• Facilitate communication between patients, families, healthcare providers, insurers, and community resources to optimize care and support.

• Educate patients and their support networks about mental health conditions and available resources, including referrals to additional services like psychiatrists and group support.

• Provide crisis intervention and psychosocial support as required, including bereavement counseling.

• Document all interactions and treatment plans accurately using our integrated healthcare software, ensuring compliance with clinical standards and regulations.

• Act as an advocate for patients and their families, enhancing communication and addressing any barriers to care.

• Monitor at-risk patients for potential delays in treatment, readmission, or complex discharge processes, coordinating solutions with care leaders.

Qualifications\n
