This position is primarily remote, granting flexibility while allowing you to positively impact clients from the comfort of your own environment. Your role will involve collaborating closely with our hospice facility’s interdisciplinary team to ensure that patients receive the mental health care they need. Though working remotely, you will remain connected with colleagues through scheduled meetings and collaborative care strategies.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand the mental health needs, strengths, and resources of hospice patients and their families, developing insights that inform tailored therapeutic interventions.

• Deliver high-quality individual, family, and group therapy, making use of evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care to support patients in their unique journeys.

• Formulate, implement, and regularly adjust individualized treatment plans and goals based on ongoing assessments, ensuring responsiveness to patients’ evolving needs.

• Collaborate with psychiatrists, psychologists, and other healthcare professionals, contributing to comprehensive care by sharing insights and creating cohesive treatment strategies.

• Facilitate crisis interventions and conduct risk assessments, providing necessary safety planning as patients navigate significant emotional challenges.

• Maintain accurate documentation in accordance with ethical and legal standards, ensuring compliance and the highest quality of care.

Patient Population / Client Focus\n

In this role, you will primarily serve individuals facing terminal illnesses and their families, supporting them through the emotional complexities of hospice care. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required, providing the foundational knowledge to engage effectively with clients.

Compensation and Benefits\n
