Key Responsibilities\n

• Conduct comprehensive psychosocial assessments via virtual sessions, identifying clients’ strengths and challenges related to their mental health needs.

• Deliver evidence-based therapeutic interventions, utilizing modalities suitable for hospice care, to individuals, families, and groups aimed at enhancing coping skills and emotional resilience.

• Collaborate with interdisciplinary teams, including physicians, nurses, and other health professionals, to coordinate integrated care plans that support the clients’ overall wellness and address their unique circumstances.

• Advocate for clients by connecting them to relevant community resources and ensuring they access the support services needed during their hospice experience.

• Maintain meticulous records of client interactions, treatment progress, and outcomes in our electronic health record system, ensuring compliance with ethical standards and organizational protocols.

• Engage actively in team meetings and case conferences, as well as pursue continuing education activities, to stay current with clinical practices and enhance your skill set.

Your work will primarily support individuals and families facing significant mental health challenges related to terminal illnesses, coping mechanisms, and end-of-life issues. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• A minimum of two (2) years of post-licensure experience providing clinical social work services with a focus on mental health.

• Demonstrated proficiency in conducting diagnostic assessments and developing individualized treatment plans that address the specific needs of clients.

• Experience with electronic health record systems and telehealth platforms for providing care and managing client records effectively.

Compensation and Benefits\n
