Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to develop tailored treatment plans for hospice patients and their families.

• Collaborate with interdisciplinary teams, including physicians, nurses, and spiritual care advisors, to ensure holistic patient care in accordance with hospice principles.

• Provide emotional support and crisis intervention, helping clients and their families cope with grief, loss, and mental health challenges.

• Educate families about the dying process, available resources, and avenues for support, empowering them during challenging times.

• Maintain accurate documentation of client interactions, treatment plans, and progress notes in compliance with state regulations and company policies.

Your primary focus will be on patients receiving hospice care—individuals who are at the end stages of life and may face various complex emotional, social, and psychological issues. You will engage with not only the patients but also their family members, providing a crucial support system as they navigate their journey. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n
