Key Responsibilities\n

• Conduct comprehensive assessments to determine client needs and tailor treatment plans accordingly.

• Regularly communicate with supervisors, family members, and relevant guardians to discuss client progress and address any emerging concerns.

• Create and present a tentative schedule detailing appointments, home visits, and professional observations to effectively monitor client advancement.

• Coordinate and execute in-home visits as necessary to conduct observations, chart reviews, and staff interviews contributing to accurate assessments and program adjustments.

• Ensure the execution of treatment plans is effective, monitoring adherence and making adjustments as needed for optimal client outcomes.
\n
• Engage actively in interdisciplinary team meetings to foster collaboration and share insights regarding client development.
\n
• Document monthly and quarterly progress notes to track client development, ensuring efficient communication and comprehensive care.
\n
• Organize and conduct training sessions for team members to ensure thorough understanding and implementation of program plans and documentation protocols.

• Provide ongoing support through follow-up assessments, including home visits and interviews to facilitate sustained progress for clients.

• Act as a bridge for the community, liaising between clients and mental health resources to enhance support systems around our clients.

• Assist in developing and refining training materials that improve program effectiveness for client care.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• A strong track record in specialized areas such as family therapy, couples counseling, trauma-informed care, or substance abuse counseling is highly preferred.
