This position offers the flexibility of remote work, making it easier to manage client visits and personal schedules. As a part of our hospice center, you will primarily focus on providing care from the comfort of your home, while still participating in team meetings and collaborative efforts to ensure holistic care for our clients. Occasionally, you may need to meet clients in their homes or at community locations based on healthcare needs.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for seniors, evaluating their emotional, mental, and social needs.

• Collaboratively develop personalized care plans with clients and their families to address specific goals and preferences.

• Provide individual and family counseling to assist seniors in navigating transitions, such as health changes or loss of independence.

• Advocate for the rights and access of clients to necessary community resources, ensuring they receive adequate support.

• Work in partnership with healthcare professionals, including nurses and caregivers, to coordinate care plans and enhance service delivery.

• Perform regular client visits to monitor emotional and physical well-being, assessing living conditions and addressing safety concerns.

Qualifications\n

Compensation and Benefits\n
