Key Responsibilities\n

• Conduct thorough psychosocial assessments through direct interviews, observational techniques, and comprehensive analysis of medical and educational records.

• Provide therapeutic counseling and emotional support to students and their families, addressing various adjustment challenges, including anxiety, grief, and coping mechanisms.
\n
• Work collaboratively with the multidisciplinary rehabilitation team, including educators, healthcare providers, and specialists, to design individualized treatment plans that address emotional and social factors impacting students’ learning and adaptation.

• Facilitate access to community supports and resources, including support groups, vocational rehabilitation, and educational assistance, to enhance students’ integration into school and community life.

• Participate actively in discharge planning and assist with the transition process for students moving from rehabilitation to standard educational settings.

• Monitor students’ progress and modify treatment plans accordingly to ensure alignment with their evolving needs.

Patient Population / Client Focus\n

Your primary focus will be on a diverse population of students requiring emotional and social support, including those dealing with adjustment issues following medical interventions or personal challenges. By addressing their unique needs, you will facilitate their successful reintegration into school and community environments.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).
