Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, adolescents, families, and groups, evaluating their emotional and psychological needs to inform the development of personalized treatment plans.

• Facilitate initial assessments of patients and their families, triaging care thoughtfully to ensure each individual receives the appropriate level of support.

• Maintain clear communication with team members about patient assessments, treatment interventions, and care plans through diligent documentation and regular meetings, including interdisciplinary patient rounds.

• Provide effective case management for assigned patients, collaborating with both healthcare professionals and community resources to create comprehensive care strategies.

• Document patient progress and treatment outcomes in a timely and ethical manner, adhering to organizational standards and ensuring compliance with all relevant regulations.

• Employ evidence-based treatment modalities, including Cognitive Behavioral Therapy (CBT), to guide patients toward the successful completion of their individualized service plans.

The clients you will support encompass a diverse range of backgrounds and age groups, including individuals struggling with anxiety, depression, trauma, and relationship issues. Qualifications\n

• Valid licensure as a Mental Health Counselor (LMHC), Marriage and Family Therapist (LMFT), or Clinical Social Worker (LCSW) in the State of New York is mandatory.

• A Master’s degree or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology is required.

• A willingness to engage in 10 to 40 hours (or more) of clinical sessions per week.

• A minimum of 2 years of clinical experience providing mental health counseling is required.
