• Actively participate in Interdisciplinary Team Meetings and contribute insights that shape patient care strategies.

• Document monthly and quarterly progress notes detailing patient advancements and areas needing attention.

• Organize and lead training to ensure that team members are fully competent in implementing care programs and maintaining comprehensive documentation.

• Offer ongoing follow-up interviews, home visits, and observations to provide continuous support aligned with each individual's evolving needs.

• Maintain organized records ensuring that all relevant information is easily accessible to staff and interested parties.

• Act as a liaison within the mental health community and the public, providing support and insights to help develop community mental health goals.

Compensation and Benefits\n

The salary for this position ranges from USD $90,000 to $100,000 per year, based on your experience and caseload capacity.
