The Outpatient clinics at Grandview Rehabilitation Center offer a dynamic and engaging work environment located in the heart of Woodbridge. Key Responsibilities

• Conduct thorough evaluations, including the review of patient history and performing specific assessments to formulate appropriate treatment plans.
• Communicate evaluation findings and treatment plans effectively to patients and their families, ensuring they are actively involved in the goal-setting process.
• Demonstrate proficiency in the implementation of customized treatment protocols based on the latest evidence and individual patient needs.
• Consult with other healthcare professionals regarding patient care modifications and address any changes in patient conditions promptly.
• Participate in discharge planning and ensure appropriate follow-up care through detailed documentation that reflects patient progress.
• Manage time efficiently to meet productivity requirements, maintaining accurate scheduling and punctual attendance at all team meetings.
• Engage in marketing initiatives that promote our services within the community.
• Contribute proactively to departmental goals and compliance with regulatory standards.

Patient Population / Client Focus

You will primarily work with patients facing a variety of orthopedic conditions, including post-operative recovery, sports injuries, and chronic pain management. Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.
