Key Responsibilities\n

In your capacity as an LPC at Jewish Family Care Therapy, you will take on a varied scope of responsibilities, including but not limited to:

• Conducting thorough and ongoing assessments to evaluate clients' mental health needs and tailor treatment plans accordingly.

• Engaging in consistent communication with supervisors, direct care staff, and family members to discuss clients' progress and any necessary adjustments in care.

• Developing and sharing a tentative schedule for appointments, home visits, and observations that align with best practices in client monitoring and support.
\n
• Facilitating home visits and conducting in-home observations, chart reviews, and data analyses to ensure comprehensive treatment.

• Overseeing the effective implementation of individualized program plans and providing follow-up support via interviews and additional home visits.
\n
• Maintaining meticulous records, ensuring that all relevant information is easily available to stakeholders and fulfilling compliance obligations.

• Acting as a liaison with the public and mental health community to enhance program development and outreach efforts.
\n
• Participating in interdisciplinary team meetings to collaborate on holistic client care and program improvements.

• Creating training materials and organizing workshops aimed at equipping staff with the skills needed for effective program implementation.

• Completing monthly and quarterly progress notes to track client developments and interventions.

You will work with a broad spectrum of patients, including adults and youth, each facing unique challenges pertaining to mental health. Qualifications\n

The ideal candidate for this role will possess:

• A Master's or Doctoral degree in Counseling, Psychology, or a related discipline.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Tennessee.

• A minimum of two years of counseling experience, ideally in clinical settings.

• Demonstrable proficiency in electronic medical records (EMR) systems and other relevant digital tools.
