This is a remote position, allowing you the flexibility to provide quality care from the comfort of your own home. Key Responsibilities\n

• Conduct thorough psychosocial evaluations of assigned patients, demonstrating strong interviewing, diagnostic, and report-writing skills.

• Develop and implement individualized treatment plans that address the specific needs and diagnoses of each patient.

• Provide direct patient care through individual psychotherapy sessions, adhering to state regulations and best practices in treatment techniques.

• Facilitate family therapy sessions to meet the therapeutic needs of patients and their loved ones, utilizing a solid understanding of family dynamics and therapy principles.

• Lead group therapy sessions designed for grieving families, helping them cope with loss and build supportive relationships within the group.

• Complete timely and accurate progress notes and assessments, in compliance with organizational procedures and documentation standards.
\n
• Engage in effective discharge planning, collaborating closely with patients, families, nursing staff, and attending physicians to ensure smooth transitions and appropriate resource utilization.

• Stay informed about community resources and referral processes to assist patients and families in accessing necessary support services.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, addressing a variety of psychological and emotional needs that arise at the end of life. You will support individuals grappling with terminal illnesses, aiding them in coping with their conditions while also addressing the emotional strains experienced by family members. Qualifications\n

• Master's degree in Social Work from an accredited institution, recognized by the Council on Social Work Education.

• Active Licensed Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences.

Compensation and Benefits\n
