Work Environment

As a Physical Therapist at our home care facility, you will work on-site, delivering care directly to patients in their homes. Key Responsibilities

• Apply evidence-based treatment protocols tailored to each patient's condition, while being responsive to changes in their health status and adjusting care plans as needed.
• Collaborate with a multidisciplinary team of healthcare providers to address complex cases, documenting any significant changes in patient conditions that necessitate modifications to the treatment approach.
• Manage discharge planning and follow-up care, ensuring thorough and timely documentation of interventions, progress notes, and discharge summaries that reflect the full course of care.
• Demonstrate effective time management skills, keeping to a structured schedule while meeting productivity expectations and maintaining punctuality for all appointments and meetings.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
