Work Environment

This position will primarily involve providing onsite care within patients' homes, which allows for personalized treatment in a comfortable, familiar setting. Key Responsibilities

• Conduct thorough examinations and evaluations of patients, analyzing their functional limitations and disabilities to develop effective treatment plans tailored to individual needs.
• Provide ongoing consultations to other healthcare providers regarding a patient's continuum of care, ensuring that all treatment strategies are coordinated and effective.
• Perform regular assessments, implementing treatment and discharge plans that align with established productivity standards while educating patients about their rehabilitation journey.
• Maintain adherence to applicable laws, regulations, and professional documentation standards while continuously evaluating treatment results and adjusting plans as necessary.
• Foster collaboration within the interdisciplinary treatment team, engaging with referral sources and physicians to optimize patient outcomes.
• Participate in community outreach initiatives and develop educational programs that promote wellness and physical therapy awareness.
• Attend professional meetings and educational programs to enhance clinical skills, ensuring the highest standards of care are upheld.

Patient Population / Client Focus

This role will involve working with a diverse patient population, primarily focusing on individuals who require home health services due to orthopedic conditions, post-surgical care, or various chronic health issues. Our team prioritizes patient-centered approaches, adapting treatments to reflect the unique cultural and personal backgrounds of each client, thereby expanding our impact within the communities we serve.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state license to practice Physical Therapy along with BLS (Basic Life Support) certification.
• Demonstrated proficiency in written and verbal communication, along with basic computer skills including familiarity with Electronic Health Records (EHR) systems.

Preferred Experience

While a minimum of two years of experience in a similar role is required, candidates with previous home care experience or those who have worked within interdisciplinary teams will be at a distinct advantage. Compensation and Benefits
