As a vital member of our home care team, you will be responsible for delivering specialized physical therapy services that promote the recovery and well-being of our patients. Work Environment

This position is fully onsite, where you will provide home care services directly to patients in their residences. Working in a home environment allows for tailored treatments that take into account each patient's living circumstance, enabling effective rehabilitation. The hours for this role are set from 9 AM to 5 PM, Monday through Friday, ensuring a structured work-life balance while being responsive to patient needs.

Key Responsibilities

• Assess patients' physical conditions, mobility, and limitations to develop tailored treatment plans that align with their functional goals.
• Implement individualized therapy sessions using a range of manual therapy modalities and rehabilitation interventions to enhance motor skills and physical performance.
• Educate patients and their families on exercises and techniques they can practice outside of therapy sessions to facilitate recovery.
• Document each patient's treatment progress, responses to interventions, and outcomes accurately and thoroughly using Electronic Medical Records (EMR) software.
• Collaborate with other healthcare professionals to ensure comprehensive care and continuity for patients, often participating in interdisciplinary team meetings.
• Continuously evaluate and adjust therapy programs based on patients' shifting needs and recovery progress.
• Foster a supportive therapeutic relationship with patients, motivating them to adhere to their programs and achieve their therapy goals.
• Encourage a healthy lifestyle by designing fitness and wellness programs that adapt to each patient's specific health status and potential.

Patient Population / Client Focus

Our home care services cater to a diverse patient population, primarily focusing on individuals recovering from surgeries, injuries, or chronic health conditions who require assistance in achieving mobility within their familiar surroundings. We also serve elderly patients requiring rehabilitation to maintain or improve their physical function, enhancing their independence and quality of life.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• Active and unrestricted license to practice as a Physical Therapist in the state of Florida.
• Demonstrated proficiency in various manual therapy techniques and rehabilitation methodologies.
• Ability to develop and implement effective therapy protocols tailored to individual patient needs.
• Familiarity with EMR systems, particularly WebPT, to maintain accurate patient documentation.
• Strong commitment to continuing education and staying abreast of current physical therapy research and techniques.

Preferred Experience

• Experience working in outpatient settings or home healthcare environments is preferred but not required.
• Proven track record of successfully guiding patients through individualized care plans.

Compensation and Benefits

You can expect a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with your qualifications and experience.
