Work Environment

This role will be performed onsite and will involve visiting clients in their homes throughout the Sanford, Florida area. Key Responsibilities

• Conduct thorough evaluations to establish a client's physical therapy needs, utilizing a variety of diagnostic and prognostic tests.
• Design and implement tailored treatment plans aimed at restoring function, improving mobility, and alleviating pain.
• Administer therapeutic exercises and treatments, and educate clients and their families about the healing process.
• Evaluate home environments to recommend modifications or assistive devices that promote safety and independence.
• Collaborate closely with physical therapy assistants, providing guidance and oversight to ensure adherence to the treatment plan.
• Maintain accurate and timely clinical documentation including initial evaluations, care plans, progress notes, and discharge summaries.
• Actively participate in interdisciplinary team meetings to coordinate comprehensive care strategies for clients.
• Monitor client progress and communicate with healthcare providers and case managers regarding any changes in condition or required modifications to the care plan.
• Educate clients and their families about the use and care of therapeutic devices and promote adherence to therapy regimens.
• Complete other assignments as directed to support the overall objectives of the organization.

Patient Population / Client Focus

As a Physical Therapist, you will focus on delivering care to diverse individuals, assessing their unique needs in their own homes. Qualifications

• A Bachelor's Degree in Physical Therapy or a similar discipline from an accredited program.
• Current state licensure as a Physical Therapist in Florida, with a commitment to maintaining good standing.
• One (1) year of experience in a physical therapy role, particularly in home health or an acute care setting.
• CPR certification and knowledge of federal and state healthcare regulations.
• Exceptional interpersonal and communication skills, fostering strong relationships with clients, families, and team members.

Preferred Experience

Compensation and Benefits
