Work Environment

This position involves onsite home care, where you will provide personalized rehabilitation services directly within patients' living environments. The role will mainly involve working Monday through Friday, with a standard 40-hour workweek. Key Responsibilities

As a Physical Therapist at Concentra Rehabilitation Center, you will play a crucial role in the following:

• Conducting comprehensive evaluations and assessments to identify the physical therapy needs of each patient referred by their physicians.
• Developing, implementing, and adjusting treatment plans based on individual assessments and evidence-based practices.
• Providing therapeutic interventions to patients, including manual therapy, exercise prescription, and patient education aimed at promoting recovery and enhancing function.
• Maintaining accurate and up-to-date documentation concerning patient progress and outcomes in accordance with regulatory requirements.
• Collaborating with fellow healthcare providers and caregivers to optimize patient recovery and ongoing care.
• Meeting established utilization and business goals to contribute positively to the organization's performance.

Patient Population / Client Focus

Your responsibilities will include treating a diverse patient population, including those recovering from orthopedic injuries, pediatric conditions, neurological disorders, and sports-related ailments. Each treatment plan you create will be tailored to the specific needs of each patient, ensuring comprehensive care that addresses their unique challenges and promotes long-term success. You will educate patients and their families on therapeutic exercises and lifestyle changes to maximize their recovery outside of therapy sessions.

Qualifications

The following qualifications are required for this role:

• Bachelor's degree in Physical Therapy from a program accredited by CAPTE or APTA.
• A valid Physical Therapist license to practice in the State of Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong written and verbal communication skills with an ability to foster connections with patients and coworkers alike.
• Proficient word processing and electronic documentation skills.

Preferred Experience

While a minimum of two years of experience as a physical therapist is required, candidates with experience in home healthcare or outpatient settings will have a competitive advantage. Compensation and Benefits
