This position entails working in a home care setting across Sanford, where you will engage with patients in the comfort of their own homes. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, needs, and treatment objectives. Assess movements, strength, and range of motion to develop individualized therapy plans.
• Create comprehensive treatment programs that utilize various rehabilitation techniques tailored to the patient's specific requirements and goals.
• Implement and supervise therapeutic interventions, including exercises and manual therapies, to optimize patient recovery and enhance functional abilities.
• Monitor and document patient progress meticulously using WebPT EMR Software, ensuring that treatment outcomes are effectively communicated and adjustments are made as necessary.
• Advise patients on proper techniques and modifications to their home environment to facilitate recovery and prevent re-injury.
• Work collaboratively with other healthcare professionals to provide a multidisciplinary approach to patient care including consultations with physicians and occupational therapists.
• Maintain a manageable caseload of no more than one patient per hour, allowing for focused and dedicated care.
• Encourage a healthy and active lifestyle through motivational support and education about wellness and fitness.
• Enhance patient treatment outcomes by efficiently transitioning them from interventions to independent home activities.
• Thoroughly document all aspects of patient care, ensuring compliance with relevant regulations and standards.

Patient Population / Client Focus

In this role, you will primarily work with a diverse orthopedic patient population, including individuals recovering from surgeries, injuries, and chronic conditions. Your expertise will directly impact patients' abilities to regain mobility and function while ensuring a high standard of care. You will have opportunities to form meaningful relationships with your clients, understanding their unique challenges and guiding them toward recovery.

Qualifications

• Valid Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active and unrestricted license to practice as a Physical Therapist in Florida.
• Strong clinical competencies and a clean professional record with no history of disciplinary actions.
• Proficient in orthopedic assessment and manual therapy techniques.
• Excellent skills in developing and executing tailored exercise protocols for rehabilitation.
• Ability to utilize electronic medical record systems, specifically WebPT, for efficient documentation of patient interactions.

Preferred Experience

• Previous experience in an outpatient orthopedic setting, particularly in home care environments, is highly preferred.
• Demonstrated knowledge and experience with evidence-based treatment methodologies.
• Strong background in alternative therapies, pain management techniques, and wellness promotion.
• Excellent interpersonal communication skills, with the ability to motivate and educate patients effectively.

Compensation and Benefits

The salary range for this role is competitive, ranging from USD 80,000 to 100,000 annually.
