Key Responsibilities\n

• Conduct ongoing and comprehensive assessments to adequately meet the needs of each client served.

• Facilitate open communication among supervisors, direct care staff, and family members to discuss progress and treatment plans for clients.

• Prepare a tentative schedule of appointments and home visits, ensuring all necessary observations for measuring work progress are included.
\n
• Coordinate and perform home visits to fulfill requirements for in-home assessments, chart reviews, and data analysis.
\n
• Create and implement effective program plans tailored to clients’ needs, evaluating their effectiveness consistently.
\n
• Participate in interdisciplinary team meetings and additional relevant sessions to collaborate on client treatment strategies.
\n
• Document monthly and quarterly progress notes, ensuring all updates reflect clients' evolving needs.
\n
• Organize and lead training sessions for staff to ensure they are proficient in implementing program plans and maintaining accurate documentation.

• Provide ongoing follow-up through interviews and home visits to facilitate continuous support for client needs.
\n
• Act as a liaison with the public and mental health community, fostering collaborative development goals.

• Assist in the development of training materials and procedures aimed at improving the quality of care provided to clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Washington.
