Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and their families, which encompass evaluating social support systems, identifying community resources, and understanding coping strategies.

• Provide crisis intervention and short-term counseling to patients and families during times of acute emotional distress, equipping them with the necessary skills and methods to manage stress.

• Offer ongoing supportive counseling from the moment of diagnosis through the progression of illness, ensuring that both patients and their loved ones feel supported and understood.

• Actively explore and connect patients and families to appropriate community resources, enhancing their quality of life and facilitating access to essential services.

• Maintain compliance with the organization’s Code of Conduct and ethical standards, addressing any concerns promptly while fostering an atmosphere of integrity.

• Monitor and document patient progress and treatment plans in our electronic health record (EHR) system, ensuring all interventions are captured effectively.

• Participate in team meetings and contribute to continuous improvement initiatives within the hospice program.

Patient Population / Client Focus\n

This role specifically supports a diverse patient population, including cancer patients and their families, who are experiencing significant psychological distress due to life-limiting diagnoses. You will be instrumental in facilitating conversations around sensitive topics, including end-of-life care, grief, and chronic illness management. Through your support, you will help patients find comfort, resolve conflicts, and create meaningful relationships with their loved ones.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in Pennsylvania is required.

• A minimum of 2 years of experience providing clinical social work support, preferably in a healthcare or hospice setting.
