Key Responsibilities

• Conduct comprehensive evaluations by reviewing pertinent health history, performing thorough assessments, and formulating tailored treatment recommendations, while documenting results effectively.
• Communicate evaluation findings and collaboratively develop personalized treatment plans with patients and their families, fostering active participation in the goal-setting process.
• Implement treatment protocols adeptly based on patient evaluations, considering their presenting conditions and utilizing the latest advancements in physical therapy.
• Consult with multidisciplinary healthcare teams as required, identifying and documenting any changes in patients' conditions that necessitate modifications to their care plans.
• Oversee successful discharge planning and follow-up care, ensuring accurate and timely documentation of all patient activities, including evaluations and progress reports.
• Maintain an effective time management strategy to meet departmental productivity benchmarks while sustaining open channels of communication.

Qualifications

• Current Basic Life Support (BLS) certification from the American Heart Association is necessary.

Compensation and Benefits
