This position is based onsite at our state-of-the-art rehabilitation facility located in Port Charlotte, Florida. In your role, you will collaborate closely with other healthcare professionals to ensure a seamless continuum of care, contributing to a truly multidisciplinary approach to rehabilitation.

Key Responsibilities

• Conduct thorough evaluations of patients by reviewing their medical history, performing specialized assessments, and documenting the findings to devise tailored treatment plans.
• Clearly communicate evaluation findings and treatment plans to patients and their families, actively involving them in the goal-setting process to ensure their engagement and compliance.
• Implement appropriate treatment protocols based on individual patient needs and reactions to therapy, adjusting methodologies as required while staying informed on the latest advancements in health care.
• Engage in consultations with other healthcare professionals, recognizing changes in patient conditions that necessitate modifications to the therapy plan, and ensuring appropriate referrals for medical or additional rehabilitation consultations.
• Oversee discharge planning and follow-up care to ensure all patient needs are met upon leaving our facility, and maintain accurate, timely documentation of all patient care activities.
• Maintain an organized schedule, showcasing exemplary time management skills by meeting productivity expectations and ensuring timeliness in administrative duties.
• Participate in community outreach and marketing activities to promote our services and enhance access to care.
• Contribute actively to departmental goals and objectives while adhering to all regulatory and accreditation requirements.
• Engage in continuous professional development and pursue educational opportunities to enhance skills, serving additionally as a clinical instructor for students in training.

Patient Population / Client Focus

Our patient population consists of individuals seeking rehabilitation for a variety of conditions, including post-surgical recovery, chronic pain, sports injuries, and age-related mobility issues. With a focus on delivering customized therapy plans, our Physical Therapists play a critical role in motivating patients through their recovery journeys and ensuring they achieve their functional goals.

Qualifications

• A Bachelor's degree in Physical Therapy (a Doctorate degree is preferred) and a valid state license to practice as a Physical Therapist in Florida.
• Current certification in Basic Life Support (BLS) from the American Heart Association for Healthcare Providers.
• Strong communicative abilities, both verbal and written, to effectively interact with patients and multidisciplinary teams.
• A commitment to patient-centered care, demonstrating empathy and understanding in all interactions.

Preferred Experience

Compensation and Benefits
