Position Overview

Pro Active Physio Clinic is seeking a passionate and dedicated Physical Therapist (PT) to join our team in Hayward, California. Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations to identify patient needs and rehabilitation goals.
• Develop and execute personalized treatment plans based on patient assessments, including therapeutic exercises, manual therapy techniques, and modalities.
• Regularly reassess patient progress and modify treatment plans as necessary to ensure optimal outcomes.
• Maintain accurate documentation including SOAP notes, progress reports, and care plans using our electronic health record system, ensuring compliance with organizational standards.
• Educate patients and their families about the rehabilitation process, addressing any concerns or questions they may have.
• Meet minimum productivity goals and contribute to the clinic's overall business objectives.
• Collaborate with colleagues across disciplines to provide integrated care for patients receiving multiple services.

Patient Population / Client Focus

You will work with a diverse range of patients, including children, adults, and elderly individuals from various backgrounds and with a variety of diagnoses. Qualifications

• A Bachelor's degree in Physical Therapy or a related field from an accredited institution is required.

Compensation and Benefits
