Key Responsibilities\n

• Facilitate telehealth sessions as part of an 8-week evidence-based program, ensuring participants receive consistent and effective care.

• Monitor, assess, and document participant progress, providing timely follow-up communications and recommendations for care coordination.

• Actively engage in collaborative care team discussions, offering insights and support in identifying resource needs for participants.

• Respond to high-risk encounters with participants following established protocols and intervention strategies to ensure patient safety.

• Prepare for and participate in team meetings, including regular check-ins with supervisors and monthly team gatherings to discuss progress and best practices.

• Adhere rigorously to HIPAA Privacy & Security policies, maintaining confidentiality and ensuring compliance with ongoing training requirements.

Patient Population / Client Focus\n

You will work directly with a diverse patient population who require support in managing various mental health challenges. Our clients include individuals dealing with stress, anxiety, depression, trauma, and other behavioral health issues. The goal is to help them access actionable skills through evidence-based practices such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), fostering resilience and improving their quality of life.

Qualifications\n

• Valid state licensure: Current Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), or a Doctor of Psychology (PsyD).

• A commitment to maintaining a consistent schedule that accommodates patient appointments effectively.

• Demonstrated clinical experience in psychotherapy, particularly using CBT, DBT, and solution-focused approaches.
