Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families, identifying needs and developing tailored care plans to enhance their psychosocial well-being.

• Engage in the delivery of graduate-level clinical social work and counseling interventions, adapting strategies based on individual client requirements.

• Facilitate effective communication with the clinical team regarding patient assessments, interdisciplinary care plans, and treatment interventions through diligent documentation and participation in team meetings.

• Ensure timely and ethical completion of all case documentation, maintaining high standards consistent with departmental expectations.

• Provide ongoing case management, collaborating with healthcare providers and community resources to ensure comprehensive care delivery.

• Deliver evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT) to assist clients in achieving their individualized service objectives.

Patient Population / Client Focus\n

You will primarily work with adults facing terminal illnesses and their families, delivering compassionate care focused on emotional, psychological, and social aspects of health. The hospice setting requires sensitivity and adaptability, allowing you to guide clients through end-of-life challenges while supporting their loved ones in navigating this journey with dignity and respect.

Qualifications\n

• A current and valid license as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or a Doctor of Psychology (PsyD) .

• A Bachelor's degree in social work, psychology, or a related field.
\n
• A minimum of 2 years of relevant clinical experience is required.
\n
• Ability to work flexible hours, ranging between 10 to 40 hours per week, accommodating varying patient caseloads.

Preferred Experience\n

While not mandatory, experience in hospice or palliative care, as well as familiarity with various therapeutic modalities and case management, will be beneficial. Experience in working with multidisciplinary teams is also a plus, as collaboration is integral to effective patient care.

Compensation and Benefits\n
