Work Environment\n

This role is designed for a remote work setting, allowing for flexibility while providing essential support to our clients. Although the majority of your work will be performed from home, you may occasionally coordinate with multidisciplinary teams, participate in virtual meetings, and engage with clients through telehealth platforms. Our flexible scheduling accommodates the varying needs of our patients, including some evening and weekend hours to ensure accessibility.

Key Responsibilities\n

• Utilize your advanced education and clinical training to make independent psychosocial assessments and diagnoses for patients coping with life-limiting conditions.

• Collaborate closely with patients to co-create individualized, patient-centered treatment plans that address their unique emotional, psychological, and social needs.
\n
• Implement evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and behavioral activation to foster patient resilience and coping strategies.
\n
• Work in conjunction with a variety of healthcare professionals, including physicians, psychologists, and other specialists within Walmart Health & Wellness to promote an integrated approach to holistic care.
\n
• Track and analyze patient progress through systematic monitoring, utilizing both in-person and virtual visits to evaluate the effectiveness of treatment and adapt plans as necessary.
\n
• Provide support for medication management in coordination with prescribing medical providers, focusing on treatment adherence, potential side effects, and overall efficacy of therapies.

Patient Population / Client Focus\n

The patients you will serve include individuals facing terminal illness and their families, requiring not only clinical expertise but also profound empathy and understanding. You will be part of a team that fosters comfort and dignity for patients during their final stages, helping to navigate the complex emotional terrain associated with end-of-life care.

Qualifications\n

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) is mandatory.

• A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee is required.
\n
• At least one year of professional experience as an independently licensed therapist, preferably with a focus on hospice or palliative care.

Preferred Experience\n

While not required, candidates with additional experience in end-of-life care, working with diverse patient populations, and familiarity with telehealth platforms will be strongly considered. Knowledge of community resources and support networks for patients and families is advantageous.

Compensation and Benefits\n
