Key Responsibilities\n

• Engage with patients in meaningful ways to assess their mental health needs and collaboratively develop personalized care plans.

• Implement evidence-based therapeutic interventions and utilize established social work theories to address client challenges and facilitate progress.

• Monitor patients’ psychosocial health and provide necessary adjustments to care strategies, particularly in response to treatment like dialysis.

• Conduct goal-oriented counseling sessions for patients needing educational or vocational rehabilitation support.

• Facilitate access to community resources, including home health services and vocational rehabilitation programs, to enhance patient outcomes.

• Assist patients and their families with advance care planning by discussing and documenting directives and preferences.

• Participate actively in interdisciplinary team meetings to share insights and updates on patient progress.

Patient Population / Client Focus\n

In this role, you will work primarily with diverse adult patients facing various mental health challenges, including those undergoing medical treatments such as dialysis and organ transplants. You will engage with clients who may also benefit from vocational and educational support. Your efforts will contribute significantly to their psychosocial adjustment and overall quality of life.

Qualifications\n

• Master's degree in Psychology, Counseling, Social Work, or a related discipline.

• Current state licensure as a mental health counselor (LMHC, LMFT, LICSW) as mandated by Washington regulations.

• Eligibility to work in the United States.

Compensation and Benefits\n
